Accident / Incident Report Guidelines
St. Albert Soccer Association recommends that the following procedure is used when dealing with an Accident or Incident involving a player or coach.
- Stay calm but act swiftly and observe the situation.
- Is there danger of further injury?
- Listen to what the injured person is saying.
- Alert the parent/guardian on site of your concern (concussion, severe sprain or injury, etc).
- In the event of serious injury, call emergency services.
- Do not move someone with serious/major injuries. Wait for emergency services to arrive.
- Deal with the rest of the group and ensure that they are adequately supervised and are safe.
- Contact the injured person’s parent/guardian if not on site.
- Advise to fill out the ASA Insurance Claim policy # 25345A
- All players and coaches are covered through registration fees.
- Complete an Accident / Incident Report (below).

Emergency Action Plan
- Dial 9 for an outside line
- Dial 9-1-1
- Give your name and suspected injury
- Give location and address

- ST. ALBERT SOCCER PLAYER DEVELOPMENT CENTER, 19 RENAULT CRESCENT, ST. ALBERT
- Tell them that someone will meet them
- At the gate of the south end parking lot - 61 Riel Dr.
- Outside the front door of 19 Renault Cr.
- Ask for the estimated time of arrival
- Report immediately to injury site
- Inform charge person that an ambulance is on the way
- Go to the ambulance meeting site
- Closest hospital:
- Sturgeon Community Hospital
- 201 Boudreau Rd.
- Closest hospital:
AED at Riel Park, located outside the double blue doors lower level at the rear of the building
